Primary Care

The Board-Certified Family Nurse Practitioner at New Leaf Primary Care Primary care provide care at every age and stage of life.

Primary care serves as the foundation of healthcare, offering comprehensive and continuous medical care to individuals and families across all stages of life. At New Leaf Primary Care, we provide care starting from the age of 5, ensuring that both children and adults receive the attention and support needed to maintain optimal health.

Our primary care services encompass health promotion, disease prevention, acute care, and chronic disease management. Health promotion involves routine check-ups, wellness visits, and preventive screenings to maintain good health and detect potential issues early. We also provide guidance on lifestyle modifications, including nutrition and physical activity.

Disease prevention is a vital aspect of primary care, including vaccinations, health risk assessments, and counseling on preventive strategies to reduce the risk of chronic conditions. Acute care addresses sudden or short-term health concerns, such as infections and minor injuries, with prompt diagnosis and treatment.

Chronic disease management is focused on individualized care plans to manage long-term conditions like hypertension, diabetes, and asthma. We help patients achieve better health control through medication management, lifestyle support, and regular monitoring. When specialized care is needed, we coordinate referrals to ensure continuity of care.

At New Leaf Primary Care, our goal is to provide holistic, patient-centered care, fostering long-term relationships and supporting the health and wellness of every patient we serve.

Our Approach

Our Board-Certified Family Nurse Practitioner utilizes a patient and family centered care approach.  A perspective on the interactions between patients, their families, and the healthcare providers is known as patient-and family-centered care, or PFCC. The foundation of the patient- and family-centered care paradigm is the idea that stronger relationships between patients, families, and healthcare professionals can lead to better healthcare services and outcomes.
