Behavioral Health Services That Work for You, Your Staff, and Your Residents

Let’s cut to it: You’ve probably worked with psych providers who ghost your building, toss meds at problems, and vanish faster than a pudding cup on Bingo night.

We’re not that team.

At New Leaf Behavioral Health, we specialize in providing comprehensive psychiatric and behavioral health services to skilled nursing facilities, assisted living centers, and long-term care communities across Illinois.

We understand your reality: residents with complex behavioral needs, overburdened staff, compliance pressures, and the constant balancing act of providing top-tier care. That’s where we come in.

Why Partner with New Leaf?

Our founder, Crystal Stone, PMHNP-BC, FNP-BC, CPMA, CPB, CPC, is a Psychiatric Mental Health and Family Nurse Practitioner who combines medical and mental health expertise with practical, real-world experience in long-term care.

We work directly with your nursing, social services, and therapy teams to create customized care plans that meet each resident’s needs. We’re responsive, consistent, and a true part of your interdisciplinary team.

  • Telehealth and In-Person Options
Whether we’re walking your halls or logging in remotely, we ensure your residents never miss the care they deserve. Flexible scheduling and minimal disruption to daily routines.

Consistency. Compassion. Care That Comes to You

At New Leaf Behavioral Health, we believe the best care happens face to face (boots-on-the-ground behavioral health care) not through a screen. That’s why we prioritize in-person visits to your facility, ensuring residents are seen where they feel most comfortable: right at home in their own environment.

We believe great care shouldn’t be delivered through a pixelated screen with spotty Wi-Fi and a “Can you hear me now?” every five seconds.

We personally visit each facility at least once a month and more often, if needed. Unlike other services that rotate providers or rely heavily on telehealth, your residents will see the same familiar face each and every time. No handoffs. No confusion. Just dependable, relationship-based care.  We believe continuity builds trust, and trust builds better care.  Walking your halls, collaborating with your team, and showing up is essential.

We’re not here to check a box; we’re here to enhance quality of life, reduce behavioral symptoms, improve mood, and be a trusted partner to your team. Like you, we want what’s best for the residents. That means personalized attention, consistent treatment, and a provider who genuinely cares.

Telemedicine is available, when necessary, but our heart is in showing up. Literally.

Walking your halls, collaborating with your team, and showing up when it matters.

Compliance & Documentation Experts

We know the surveyors are watching. We help ensure your psychotropic documentation and treatment plans meet CMS, state, and facility guidelines. With a certified professional biller and coder on staff, you can be confident our charting supports medical necessity and protects your facility from unnecessary scrutiny.

Let’s Clear Something Up:

We don’t slide in with empty promises, vague commitments, or sketchy contract clauses that quietly tell your medical director to back off the meds.

We’re not here to play games or trick you into signing a deal that sounds too good to be true because guess what?

It usually is.

You’ve heard it before:

🗣️ “We round weekly!” (Until they don’t.)
🗣️ “We’ll take over all your behavioral cases!” (But nobody shows up.)
🗣️ “We’ll make your psychotropic GDR compliance magically perfect!” (With a provider who visits quarterly, maybe.)

That’s not us.

We’ve seen what other psych groups do to “get in the door” with big promises, flashy sales pitches, and contract clauses sneakier than a resident hoarding pudding in their sock drawer.

Here’s what we do instead:

✅ We tell the truth.
✅ We commit to what we can actually deliver.
✅ We do not tell your staff, DON, or medical director what they “can’t touch.”
✅ We show up. We follow through. We collaborate. Every. Single. Time.

We believe in integrity over ego, consistency over chaos, and doing what we say we’ll do even if it’s not the flashiest pitch. We’d rather earn your trust than hustle you into a bad relationship.

Let’s Build Something Better—Together

We know your residents aren’t just room numbers they’re people with stories, struggles, and sometimes very strong opinions about pudding. We’re here to help them feel better, live better, and age with dignity, not to over-medicate or check out when things get complicated.

At the end of the day, we want what you want: residents who feel seen, heard, and cared for.

If you’re tired of fragmented care, outside providers who don’t understand long-term care, or just want a more collaborative partner in managing your residents’ behavioral health, let’s talk.

We’d love to learn more about your facility and how we can support you.
