Necessary Documents for Establishing Care
To begin the process, we require the following documents from you:

Referral from Your Primary Care Provider (PCP) (if applicable):

Some healthcare services may require a referral from your PCP. If needed, please ask your PCP to fax the referral to us at (618) 416-7734. The referral helps us coordinate your care efficiently.

Insurance Card:

You may send a copy of your insurance card even if the referral is not yet available. This allows us to verify your insurance eligibility in the meantime.

Personal Information:

Please provide your full name, address, and date of birth either via email or phone. This information helps us initiate the intake process and create your patient profile.

Master Plan Document / Summary Plan Description (SPD) for Commercial Insurance Holders
If you have commercial insurance, we will need the Master Plan Document (MPD) and Summary Plan Description (SPD). This document details your insurance plan’s coverage, benefits, limitations, and procedures. Unlike a simple benefits summary, the SPD provides comprehensive information that helps us understand your coverage and advocate for your benefits when necessary.
How to Obtain Your MPD/SPD:

Through Your Employer (if employer-sponsored):

Contact your Human Resources (HR) department or benefits administrator.

Request the Master Plan Document and Summary Plan Description (SPD), not just the Summary of Benefits.

Many employers provide digital access through an employee portal.

Log in to your insurance provider’s member portal and search for “Plan Documents” or “SPD.”

Some insurers have a section specifically for policy documents, where you can download the SPD directly.

The MPD/SPD helps us understand the specific details of your insurance plan, including coverage for healthcare services. Having this document allows us to advocate effectively on your behalf and ensures we can provide the care you need without unexpected coverage issues.

Scheduling Your Appointment
Once we receive your referral and verify your insurance, our staff will contact you to schedule your initial appointment. At this time, we will also send you the necessary intake forms electronically.

What If My Referral Is Not Ready Yet?

If your referral is not yet available, you can still send us your full name, address, date of birth, and a copy of your insurance card. This way, we can start the insurance verification process while waiting for the referral.

Completing Intake Forms:

To ensure your appointment will not cancelled, please complete and return all intake forms and questionnaires at least one week prior to your appointment. This allows us adequate time to review and add the information to your chart.

If you encounter any issues while completing the forms, please call our office for assistance.

Incomplete forms may result in appointment cancellation. You can reschedule after completing the forms.

If you are late or fail to show up for your first appointment (except in emergency situations), we may not be able to reschedule as per our office policy.
